Clinical Nurse Coordinator

The Clinical Nurse Coordinator provides clinical support to medical providers, providing care management to their entire panel of patients. As the primary point of contact for complex outpatients, the Lymphoma CNC provides continuity of care through nursing assessments, triage, patient education, and care coordination. The clinical nurse coordinator is an integral part of a dedicated multidisciplinary team that collaborates closely to provide the highest level of care for patients.

This is a full-time 1.0 FTE position at our South Lake Union location. The schedule for this position may vary, from Monday through Friday from 8:00 am to 4:30 pm.

Responsibilities

The Clinical Nurse Coordinator is responsible for:

• Assessing physical, emotional, social, and spiritual needs and evaluating patients' adaptation to health changes
• Developing a plan of care based on the patients' disease, symptoms, and response to treatment
• Educating patients, families, and caregivers on disease processes, treatment, side effects and symptom management
• Coordinating with the interdisciplinary team to ensure continuity of care
• Referring patients, families, and caregivers as appropriate to other clinical resources/professionals such as social work, home health care, clinical nutrition, etc.
• Implementing, evaluating, and modifying plan of care
• Integrating research and evidence-based knowledge into clinical practice

In addition to the primary responsibilities of the Registered Nurses (RN), the Clinical Nurse Coordinator (CNC) will have the following key responsibilities:

Direct Patient Care

• Educating patients and families: specific to rehabilitation services
• Telephone and email symptom triage
• Engage in meaningful patient follow-up and care coordination
• ex: post-procedure, medication side effects
• New patient orientation to the service line
• Caregiver support
• Assist with in-clinic procedures:
• (set up, consent, time out, and patient positioning)
• Assessment of needs for additional supportive care resources
• Prescription refills and addressing medication questions
• Result reviews

Indirect Patient Care

• Supportive care referrals
• FMLA paperwork, disability paperwork, Ferry passes
• Coordination of outside clinical care
• Ensuring continuity of care with outside providers
• Collaboration with the research team for patients on trial
• Place physiatry return orders and Supportive Care referral orders as directed by MD
• Coordinate responses to patient inquiries via MyChart and phone
• Coordinate lab tests and notification of patients
• Coordinate with Supportive Care and medical care teams as needed, across Fred Hutch sites
• Facilitate external referrals for ongoing rehabilitation needs
• Manage Epic Rehab Medicine Clinical Support InBasket inquiries from patients, staff, or other providers

Communication and Teamwork

• Attend regular team meetings with the provider and nursing teams
• Coordination of care with ancillary services
• Delegation of tasks to appropriate team members
• Coordinating patient schedules with the clinic team
• Collaboration with infusion RN and other members of the care team to ensure patient needs are met
• Real-time management and escalation of issues in the clinic

Professional Development and Clinic Engagement

• Document all nursing care activities accurately and in a timely fashion.
• Demonstrate knowledge and understanding of patient privacy rights.
• Active participation in continuing education, departmental projects, and committees.

Qualifications

Required:

• Graduated from an accredited school of nursing
• Must have a current Washington State Registered Nurse (RN) license or current NLC multistate license (MSL); and must upgrade to an MSL with primary state of residence, WA, within 60 days of hire. This requirement may be waived under specific circumstances, subject to approval.
• Applicants that do not have previous oncology experience will be provided additional training and support
• Current BLS AHA Health Provider Card (or equivalent course, such as by the American Red Cross) and renewal required every two years
• Additional certification may be required dependent upon the department.
• Critical thinking: the ability to practice the cognitive skills of analyzing, applying standards, discriminating, information seeking, logical reasoning, predicting, and transforming knowledge
• Ability to develop clinical judgment.
• Time Management skills: the ability to organize and manage time and tasks independently
• Ability to communicate effectively by looking and listening for cues, asking open-ended questions, exploring cues, using pauses, screening responses, and clarifying responses.
• Ability to effectively listen by using reflecting, acknowledging, summarizing, empathizing, and paraphrasing skills.
• Ability to effectively provide information by checking what information the person knows already, giving small amounts of information at a time, using clear terms and avoiding jargon, avoiding detail unless it is requested, checking for understanding, and pausing and waiting for a response.

Preferred:

• BSN-prepared nurse
• OCN (Oncology Certified Nurse) or BMT RN
• 2 years of oncology nursing experience
• Previous telephone triage experience
• Previous outpatient clinic experience
• Takes initiative
• Good communication skills
• Skilled at multitasking and organization

The hourly pay range for this position is from $47.64 to $88.19, and pay offered will be based on experience and qualifications.??Registered Nurses are paid on a step-scale basis and are also eligible for shift differentials and other pay premiums such as advanced degree, preceptor, and certification.
