Team Leader

$16.5 per hour

Personal Care Services MidSouth

Job Description

Job Description

Job Summary

The Field Staff Team Lead plays a critical role in maintaining care quality, operational compliance, and supporting field staff. You will divide your time between leading, mentoring, and supervising field caregivers and actively driving new client acquisitions through strategic community marketing. This role requires an adaptable leader who ensures operational excellence during business hours and provides seamless hands-on care coverage when needed.

Compensation & Hours Structure

  • Base Hourly Rate: $16.50 and up per hour (dependent on experience).

  • Regular Business Hours: The base hourly rate applies strictly to hours worked Monday through Friday, 8:00 AM – 5:00 PM.

  • After-Hours & Weekend Coverage: Any hands-on shift coverage or field duties performed before 8:00 AM, after 5:00 PM on weekdays, or anytime on weekends will be paid at the standard caregiver rate of that area.

Key Responsibilities

1. Field Leadership & Team Management (60%)

  • Supervise, mentor, and evaluate a team of non-medical field caregivers.

  • Review and finalize daily caregiver clock-in/out records for accuracy to ensure precise payroll and billing processing.

  • Coordinate and conduct all new hire in-home training and client introductions to ensure proper care plan implementation and client comfort.

  • Make all final decisions on caregiver assignments, determining which specific staff member is assigned to each home to optimize retention and client compatibility.

  • Conduct routine on-site supervisory visits and field introductions for caregivers and clients.

  • Act as the first point of escalation for caregiver questions and client care concerns during the workday.

  • Ensure all field staff strictly adhere to care plans, company policies, and compliance regulations.

  • Shift Coverage: Serve as a priority backup to step in and cover client shifts in the field if an immediate scheduling gap or caregiver call-out occurs.

2. Marketing & Community Outreach (40%)

  • Build and maintain relationships with key referral sources, including hospital discharge planners, rehab facilities, senior centers, and long-term care communities.

  • Conduct in-home assessments for prospective clients, present service options, and close care agreements.

  • Represent the agency at local health fairs, networking groups, and senior community events.

  • Coordinate with internal teams to align field capacity with marketing efforts.

Clear Marketing & Operational KPIs (Key Performance Indicators)

To succeed in this role, you will be evaluated on the tracking, growth, and accuracy of the following metrics:

  • New Referral Sources: Identify, establish, and onboard brand-new professional referral relationships monthly.

  • Referrals Received from Sources: Track and maintain the total volume of incoming client leads generated from your established referral network.

  • Referral Conversion to Clients: Achieve and maintain target conversion rates from initial referral/in-home assessment to signed service agreement.

  • Community Activity: Plan, execute, and document consistent local outreach efforts, including networking events, senior center visits, and health fairs.

  • Billing Source of Payer: Accurately identify, document, and verify the client's payer source (e.g., Private Pay, Medicaid Waiver, Long-Term Care Insurance) during intake.

  • Clients Record of Contact: Maintain exhaustive, up-to-date documentation of every touchpoint, phone call, visit, and update made with prospective and active clients.

  • Client Satisfaction Surveys Completed: Coordinate the distribution and collection of routine quality assurance and satisfaction surveys from active clients to drive retention.

Qualifications & Compliance Requirements

  • Licensing: Must possess an active Certified Nursing Assistant (CNA) license in good standing.

  • Certifications: Must hold and maintain a current, valid CPR certification (Basic Life Support/BLS preferred).

  • Experience: Minimum of 3 years of experience in a non-medical home care or home health setting, with at least 1 year in a supervisory or team lead experience preferred.

  • Sales/Marketing Experience: Minimum of 2 years of experience in sales (retail or healthcare), business development, or community outreach.

  • Regulatory Compliance: Full knowledge of and adherence to Mississippi State Department of Health (MSDH) compliance guidelines and regulations for personal care agencies.

  • Training & Certifications: Must meet all MS state-regulated training requirements, including the completion of state-mandated training, orientation, and annual continuing education.

  • Background Checks: Must successfully pass all MS state-required background checks, including the Mississippi standard criminal history fingerprint check, Nurse Aide Registry check, and OIG exclusion screening.

  • Transportation: Must have a valid driver’s license and a clean driving record to operate the assigned company car for all local travel, field supervision, and marketing visits.

  • Skills: Exceptional communication, conflict resolution, and relationship-building skills. Proficient with home care scheduling / Electronic Visit Verification (EVV) software and CRM tools.
