Job Details Location: UCHealth Greeley Hospital, Greeley, CO Department: GH Care Management Work Schedule: Full Time, 80 hours per 2‑week pay period Shift: Days Pay: $33.82 – $50.73 per hour, based on experience Onsite role; no hybrid or remote option Summary Assesses patients and/or family, caregivers, and/or legal representatives to arrange needed interventions. Plans interventions to address social, emotional, economic, and environmental problems and provides short‑term treatment planning for mental, emotional, behavioral disorders and addictions. Responsibilities Participates in case reviews to evaluate care management and progress toward goals. Consults with healthcare team members to promote, monitor, and evaluate compliance with the patient’s treatment plan. Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan. Periodically administers and interprets tests and measures of psychosocial functioning for diagnosing mental, emotional, behavioral, addictive and developmental disorders and disabilities, implementing an assessment‑based treatment plan. Supervises and/or trains new staff, lower‑level social workers, students or interns. Applies critical thinking, decisive judgment, and the ability to work independently in a fast‑paced environment and take appropriate action. Requirements Master’s degree in Social Work.
