What You'll Be Doing Provide advanced wound care assessments, treatment planning, and ongoing management. Conduct routine rounding visits throughout assisted living, memory care, and long‑term care communities. Manage medications, diagnostics, treatment plans, and follow‑up care. Collaborate with physicians, nurses, therapists, caregivers, and facility teams. Support chronic disease management, preventative care initiatives, and continuity of care. Help improve healing outcomes while reducing avoidable complications and hospitalizations. Qualifications Active Minnesota Certified Nurse Practitioner (CNP) license (AGNP preferred). Experience in wound care, geriatrics, primary care, post‑acute care, skilled nursing, or related settings preferred. Strong clinical judgment and confidence managing medically complex patients.
