Physical Therapist (PT) - Full-Time

Location: DeLand, FL Department: Rehabilitation Services Employment Type: Full-Time Compensation: $50-$65 per hour

Position Overview

We are seeking a compassionate and skilled Physical Therapist (PT) to join our rehabilitation team in a skilled nursing and post-acute care setting. The Physical Therapist will evaluate residents, develop individualized treatment plans, and provide therapeutic interventions designed to improve mobility, reduce pain, restore function, and enhance overall quality of life. This role offers the opportunity to work collaboratively with an interdisciplinary healthcare team while making a meaningful impact on residents' independence and well-being.

Key Responsibilities

Assessment & Treatment Planning

• Perform comprehensive physical therapy evaluations upon admission and as needed.
• Develop individualized treatment plans based on physician orders, clinical findings, and resident goals.
• Establish measurable short- and long-term rehabilitation goals.
• Reassess resident progress and adjust treatment plans accordingly.

Therapeutic Services

• Provide direct physical therapy treatments, including:
• Therapeutic exercise programs
• Gait and mobility training
• Balance and coordination activities
• Pain management techniques
• Recommend and train residents in the use of assistive devices such as walkers, wheelchairs, and braces.
• Promote functional independence and safe mobility.

Team Collaboration & Supervision

• Supervise Physical Therapy Assistants (PTAs) and delegate responsibilities appropriately.
• Provide guidance and support to therapy aides, nursing staff, residents, and family members.
• Participate in interdisciplinary meetings, care conferences, and discharge planning.

Documentation & Compliance

• Maintain accurate and timely documentation, including evaluations, daily treatment notes, progress reports, and discharge summaries.
• Ensure compliance with Medicare, Medicaid, PDPM, HIPAA, and other regulatory requirements.
• Adhere to all federal, state, and organizational policies and standards.

Resident & Family Education

• Educate residents and caregivers on therapeutic exercises, safety precautions, fall prevention, and mobility strategies.
• Communicate treatment progress and recommendations effectively with healthcare team members and families.

Qualifications

Required

• Degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license in the state of practice.
• Strong clinical assessment and treatment planning skills.
• Excellent communication and interpersonal abilities.
• Commitment to patient-centered, evidence-based care.

Preferred

• Experience in skilled nursing, rehabilitation, long-term care, or post-acute settings.
• Knowledge of Medicare, Medicaid, PDPM, and related reimbursement guidelines.
• Experience supervising Physical Therapy Assistants (PTAs).
