Case Manager: Interdisciplinary Care & Discharge Planning
PAM Health Rehabilitation Hospital of Allen
PAM Health Rehabilitation Hospital of Allen is seeking a Case Manager to coordinate interdisciplinary care for inpatient populations. You will review admission records to ensure proper utilization and determine level of care, assess discharge needs, and coordinate discharge planning. Work with therapy, nursing, social work and payors to improve outcomes and maintain payor relationships. Licensure in the state and current BLS required; 3–5 years inpatient experience preferred in acute/IRF/LTACH #J-18808-Ljbffr PAM Health Rehabilitation Hospital of Allen
