As Circadia’s on-site NP, you’ll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions - reducing preventable readmissions. You’ll blend direct patient care with tech-to-bedside integration through hands-on training, live demos, and shadowed assessments. This is a full-time, embedded NP role requiring Monday-Friday, on-site presence at our partner facility, Cedar Creek Post Acute in Mt. Juliet, TN. What You’ll Do Direct patient care: Conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers. Tech → bedside integration : Train and coach staff at the point of care, standardize use of Circadia into clinical workflows. Data into action : Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline. Close the loop : Complete documentation/charts within the same day High-volume rounding : Efficiently complete visits across designated SNFs (typ. 25+/day where appropriate), focusing on new admits, facility-flagged patients, and Circadia alerts. Quality & product feedback : Surface field insights to improve workflows and tech integration in a clinical setting Mentor & collaborate : Support NP onboarding, share best practices, and partner with clinical ops/product teams How Success is Measured (KPIs) Visits: Target 25 patient visits/day Engagement: ≥80% Time to Acknowledgement Rate: Documentation: Complete all notes using AI-powered tools within 24 hours of each encounter. Adoption & impact: Demonstrated workflow adherence at facilities; trend toward fewer falls/readmissions over time. Must-Have Qualifications Certified and licensed Nurse Practitioner in MI; ANCC (or equivalent) in Adult/Family/Geriatric/Acute. Eligible for Medicare participation; company bills on your behalf. 2+ years NP experience with complex adult populations; able to practice independently. Valid MI driver’s license; comfortable traveling locally. Thrives in a dynamic, evolving startup environment. English required Nice-to-Haves Tech-savvy: Google Suite, multiple EMRs, Slack, AI transcription/tools. Spanish is preferred. $120,000 - $140,000 a year As a full-time Nurse Practitioner , you will be employed by Circadia Health, Inc. The anticipated annual base salary range for this full-time position is $120,000 - $140,000 . The base range is determined by role and level, and placement within the range will depend on a number of job-related factors, including but not limited to your skills, qualifications, experience, and location.
