Job Description

Job Summary:
Provide skilled physical therapy services to patients in a home health setting, adhering to all clinical and safety standards. Collaborate with the healthcare team to develop and implement personalized care plans for optimal patient outcomes.

Responsibilities:

• Evaluate patient conditions and establish appropriate treatment goals.
• Develop and execute individualized treatment plans based on physician referrals.
• Monitor and document patient progress throughout therapy sessions.
• Educate patients and caregivers on exercises and home safety techniques.
• Comply with all facility policies, including attendance and documentation requirements.
• Assist with travel and equipment setup, ensuring safety and comfort during visits.

Qualifications:

• Valid physical therapy license, with pending licenses accepted for consideration.
• Minimum of 1-2 years of home health experience preferred.
• Strong knowledge of PT protocols and patient care standards.
• Ability to pass required competency tests, including cardiac dysrhythmia and sedation assessments.
• Proven ability to work independently and within scope of practice.
• Must demonstrate good communication and documentation skills.
