About the Position

Description

• Provide skilled nursing care to patients in their homes based on individualized care plans.
• Perform comprehensive patient assessments and monitor changes in patient condition.
• Complete accurate and timely OASIS assessments and documentation .
• Provide patient and family education regarding medications, treatment plans, disease management, and self-care.
• Coordinate care with physicians, caregivers, and other members of the interdisciplinary healthcare team.
• Maintain accurate clinical documentation and comply with home health regulations and agency standards.
• Manage patient visits efficiently while ensuring high-quality, compassionate care.

Requirements

Requirements

• Active Registered Nurse (RN) license .
• Previous Home Health nursing experience required.
• SHA and OASIS experience required.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong clinical assessment, documentation, and communication skills.
• Ability to work independently and manage a home health caseload effectively.
• Valid driver's license and reliable transportation preferred for home visits.
