In this key role, you will coordinate new resident admissions, perform clinical assessments, oversee care transitions, and help ensure exceptional outcomes for our residents.If you thrive in a fast-paced healthcare environment, have strong clinical assessment skills, and enjoy collaborating with interdisciplinary teams, we'd love to hear from you.Position SummaryThe RN Admissions Nurse is responsible for coordinating and facilitating the admission process for new residents while ensuring accurate clinical assessments, documentation, regulatory compliance, and seamless transitions of care. This position works closely with physicians, hospital discharge planners, therapy teams, nursing staff, residents, and families to deliver a positive admission experience and maintain high-quality patient care.Key ResponsibilitiesAdmissions & Clinical AssessmentCoordinate and manage all aspects of resident admissionsComplete comprehensive nursing assessments upon admissionReview hospital records, physician orders, medications, and treatment plansEnsure accurate and timely admission documentationVerify insurance, authorization, and required clinical informationFacilitate smooth transitions from hospitals and other healthcare settingsCare CoordinationCollaborate with physicians, nursing staff, therapy, pharmacy, and interdisciplinary teamsCommunicate admission details and care needs to appropriate departmentsEnsure timely implementation of physician orders and care plansCoordinate laboratory services, diagnostic testing, and follow-up careAct as a liaison between residents, families, and the care teamClinical Nursing SupportAssess resident conditions and identify changes in statusAdminister medications, treatments, and IV therapy as neededRespond appropriately to medical emergenciesSupport residents in achieving the highest level of independence and well-beingProvide clinical guidance and support to nursing staff during the shiftDocumentation & ComplianceMaintain accurate and complete admission recordsEnsure compliance with federal, state, and facility regulationsMonitor documentation accuracy and regulatory requirementsParticipate in quality improvement initiatives and auditsMaintain confidentiality and uphold HIPAA standardsResident & Family RelationsEducate residents and families regarding care plans and facility servicesAddress questions and concerns throughout the admission processPromote a welcoming and compassionate admission experienceMaintain professional and effective communication with residents and familiesQualificationsRequiredActive Registered Nurse (RN) license in CaliforniaGraduate of an accredited nursing programMinimum 2 years of RN experienceStrong clinical assessment and care coordination skillsExcellent communication, organization, and documentation abilitiesPreferredExperience in Skilled Nursing Facilities (SNF), Post-Acute Care, or Long-Term CareAdmissions nursing experience strongly preferredExperience with PointClickCare (PCC) or other Electronic Health Record (EHR) systemsBSN preferredWhy Join Arden Park Post Acute?Competitive pay: $48.00–$50.00 per hour DOEFull-time, stable scheduleSupportive leadership teamCollaborative clinical environmentCareer growth and advancement opportunitiesOpportunity to make a meaningful impact on resident care and outcomesAbout Arden Park Post AcuteAt Arden Park Post Acute, we are committed to delivering compassionate, high-quality skilled nursing and rehabilitation services.
