Registered Nurse (RN) Hospital Liaison

Position Registered Nurse (RN) – Hospital Liaison (post‑acute care) – Schedule: Monday–Friday 8:00am–4:30pm; on‑call weekends and ASHN observed holidays. Agency: The OSU Wexner Medical Center Home Care. Role Overview As a Post‑Acute Care Coordinator (PACC), you will provide patient‑healthcare coordination services, attending discharge and multidisciplinary rounds in acute care, ambulatory, and other health‑system settings to assist patients in the transition of care from one setting to the next. Core Responsibilities Attend discharge and multidisciplinary rounds in acute care, ambulatory and other health‑system settings. Determine home‑care eligibility and review patient insurances and medical documentation. Coordinate healthcare services as ordered by the attending physician and assist in the discharge‑planning process. Ensure coordination of all ancillary services per the patient’s needs following discharge and minimize patient risk. Promote patient well‑being and serve as a patient advocate/functional resource nurse or social worker. Increase awareness of services offered, maintain facility relationships, and build lasting positive relationships with patients/clients and facility/hospital personnel, physicians and other team members. Review and complete all clinical documentation following agency protocol and Medicare/Federal guidelines. Participate in care conferences, coordination of case management, and Care Integration meetings. Notify the referring facility manager before contacting patients. Hard and Soft Skills Compassionate communicator with a positive attitude. Patience when working with patients, families, physicians, and coworkers. Attention to detail, observant, and able to follow directions. Qualifications Registered Nurse with a current license in the state of employment. Minimum of two years of experience. Home‑care experience preferred. Valid driver’s license and auto insurance in your name as a driver. Capable of meeting all physical demands of the job.
