Work Environment

This position is primarily onsite within our Home Care facility in Fort Worth, where you will collaborate closely with a multidisciplinary team of healthcare professionals. Key Responsibilities

• Conduct thorough psychiatric assessments and comprehensive evaluations to accurately diagnose patients' mental health disorders.
• Manage and monitor medication regimens for patients, adjusting treatment plans as necessary based on patient progress and response.
• Collaborate with primary care providers and other healthcare professionals to create a cohesive treatment plan that addresses both physical and mental health needs.
• Provide crisis intervention and emotional support during acute episodes, ensuring patients receive timely and compassionate care.
• Engage in community outreach initiatives to educate the public about mental health issues and promote available services.
• Offer therapeutic support through individual or group counseling, infusing psychoeducation that empowers patients to manage their conditions effectively.
• Maintain meticulous and confidential patient records, complying with HIPAA regulations to ensure patient privacy and data security.

Patient Population / Client Focus

As a Psychiatric Nurse Practitioner at Vitality Health Clinic, you will serve a culturally diverse patient population comprising children, adolescents, and adults facing a range of mental health challenges, including anxiety, depression, PTSD, and more. Your role will extend beyond clinical diagnoses; you will advocate for your patients’ needs and connect them with essential support services to enhance their overall well-being.

Qualifications

• A Master's degree in Nursing with a specialization as a Psychiatric Nurse Practitioner is required.
• Active state licensure as an Advanced Practice Registered Nurse (APRN) with psychiatric specialization is mandatory.
• Strong communication and interpersonal skills essential for building relationships with patients and collaborating with a multidisciplinary team.

Preferred Experience

• Previous experience working in a community health center or similar healthcare setting is highly preferred.

Compensation and Benefits
