MDS RN (Registered Nurse) – Full-Time
Lebanon, OH

Are you an experienced Registered Nurse (RN) with a strong background in MDS, Medicare/Medicaid, RAI, and long-term care ? We're looking for a detail-oriented clinician who is passionate about quality care, accurate reimbursement, and making a meaningful impact on resident outcomes.

What You'll Do

• Complete accurate and timely MDS assessments in accordance with CMS guidelines.
• Coordinate the RAI process, CAAs, and care plans.
• Collaborate with nursing, therapy, dietary, social services, and physicians.
• Monitor Medicare and Medicaid documentation for compliance and reimbursement.
• Identify significant changes in resident condition and ensure appropriate assessments are completed.
• Participate in Quality Measures and Performance Improvement initiatives.
• Maintain compliance with federal and state regulations.

Qualifications

• Current Ohio RN license required.
• MDS experience in a skilled nursing or long-term care setting required.
• Strong knowledge of RAI process, Medicare, Medicaid, and CMS regulations.
• Excellent assessment, documentation, organizational, and communication skills.
• RAC-CT certification is a plus but not required.
