Our facility also qualifies as a high-need area for provider CA State and Federal Loan Repayment Programs.Reports To: Assistant Medical DirectorFLSA Status: Non-ExemptPosition Function: ClinicalClassification: TemporarySchedule: Varies, On-CallLocation: Onsite, position may be assigned to work at both IHC locationsSUMMARY:Provides general medical, preventive, acute and chronic care, and treatment to patients in medical facility such as clinic, health center, or public health agency, under direction of Physician, by performing the following duties.ESSENTIAL DUTIES AND RESPONSIBILITIES: The information in this job description indicates the general nature and level of work performed. Performs physical examinations and preventive health measures within prescribed guidelines and instructions of Physician.2. Orders, interprets, and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs.3. Discusses case with Physician and other health care professionals as needed to prepare comprehensive patient care plan.4. Prescribes or recommends drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures.5. Refers patients to Physician for consultation or to specialized health resources for treatment.6. Assists other staff members as directed by the medical director.7. Institutes referral and coordinate patient services to promote continuous comprehensive care at the primary, secondary, and tertiary levels.8. Incorporate into the plan of care provisions for the preventive measure to improve health status based upon individual, cultural, psychological, social, and environmental factors.9. Provides information for the patient, enabling patient to make realistic decisions in solving health and related problems and ensure joint responsibility with the professional health team members for the outcome of the patient’s care.10. Advises and counsels patients about normal developmental and maturational stages, and normal life crisis. Identifies and intervenes in problems related to these areas.11. Collects and records data according to the problem oriented medical record system, assuring timely and accurate completion of data entry into patient records.12. Provides support and counseling for patients with chronic disease regarding psycho-social, environmental, and physical well-being.13. Institutes emergency medical measures in life threatening situations.14. Assess and intervene in common developmental and behavioral problems of individuals in the family.15. Counsel’s men and women on available contraceptives and facilitate making the indicated birth control measures available.16. Monitors changes of patients with chronic disease.17. Prescribes clinic drugs to the patient in accordance with the formulary and policies of the health manpower pilot project 129 or under the supervision of a prescribing physician. Physician orders may be given to the FNP orally or in writing or in accordance with written standardized procedures developed under these guidelines. Attends and participates in organized meetings as requested.18. Transmits a valid prescription to a pharmacist, by telephone, or in person or by written prescription, as so authorized by the HMPP 129 or in accordance with written standardized procedures.19. Maintains patient confidentiality.20. Participates in the education of fellow staff members in the form of lunch and learn presentations as assigned by Assistant Medical Director.21. Interacts with community members via participation in local tribal health screening events.22. Position may be re-assigned during activation of EOP.SUPERVISORY RESPONSIBILITIESThis position has no supervisory responsibilities.TRAVEL & DRIVING REQUIREMENTSTravel is not required for this position.Driving may be required on occasion to participate in local tribal health screening events.EDUCATION AND/OR EXPERIENCEGraduate of an accredited school of Nurse Practitioners.Minimum of 1 year experience as a family nurse practitioner in an ambulatory center.REQUIRED KNOWLEDGE, SKILLS, AND ABILITIESKnowledge of professional nursing theory, practice, and general care to deliver and evaluate patient care. Knowledge of organizational policies, regulations, and procedures to administer patient care.Knowledge of medical equipment and instruments to administer patient care.Knowledge of common safety hazards and precautions to establish a safe work environment.Skill in applying and modifying the principles, methods, and techniques of professional nursing to provide ongoing care.Skill in taking medical history to assess medical condition and interpret findings.Skill in preparing and maintaining records, writing reports, and responding to correspondence.Skill in developing and maintaining quality assurance.Ability to react calmly and effectively in emergency situations. Ability to interpret, adapt and apply guidelines and procedures.Productivity Software: Microsoft Office 365; Word, Excel, PowerPoint, Email & Calendar Management (outlook), Teams (video conferencing and meetings), and Share Point.Electronic health records software (NextGen and Epic preferred).Utilization of Paycom, Paycom Learning, Ethic Point and PolicyTech systems.CERTIFICATES, LICENSES, MEMBERSHIPS AND REGULATIONSCurrent Family Nurse Practitioner license required.A valid California driver’s license and a good driving record will be required at the time of appointment and must be maintained throughout employment.Current BLS, ACLS, and PALS CPR certification or attainment within 6 months of hire.Applicants must successfully pass a pre-employment physical exam, tuberculin skin test or x-ray and urine drug screen test to include marijuana.Health must be adequate to perform all duties of the position.Must pass criminal background check.WORK ENVIRONMENTThe work environment characteristics described are representative of those an employee encounters while performing the essential functions of this job.Indoor clinical setting.PHYSICAL DEMANDSThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job position.
