Hospice RN Case Manager: Compassionate Care Coordinator
Affinity Hospice
Affinity Hospice is seeking an RN Case Manager to join the interdisciplinary hospice nursing team in Illinois. You will coordinate care for patients and families after identifying needs across physical, psychosocial, and spiritual domains, initiating appropriate interventions, and providing continuous, individualized care plans. On-call visits, admissions, and death visits may be required.
