Breaking Ground in New York, NY is seeking a Consumer Advocate, Outreach to engage people experiencing street homelessness with chronic medical conditions in Manhattan and help connect them to social and medical services and housing solutions. The role involves extensive field work, and the incumbent will build rapport with clients using motivational interviewing, conduct assessments, and enter data into required databases.
