Registered Nurse (RN) Case Manager

Responsibilities

Manage a defined caseload of patients within the Greater Lakes Region territory.
Provide comprehensive nursing services and direct patient care in the community setting.
Coordinate patient care, communicate with healthcare providers, and ensure care plans are effectively implemented.
Maintain accurate documentation and adhere to company policies and regulatory requirements.

Requirements

Valid Nursing License (RN) in the State of New Hampshire.
Minimum of two years acute care nursing experience.
Current CPR BLS Certification (monthly renewal classes available and paid for by Interim HealthCare).
Reliable transportation to travel within the Greater Lakes Region.
Home care experience is preferred but not required.
