Job Summary

We are seeking an experienced Registered Nurse Case Manager for a travel assignment in Southern California. This role focuses on care coordination, discharge planning, and collaborating with interdisciplinary teams to ensure optimal patient outcomes in an acute care setting.

Responsibilities

• Coordinate patient care and discharge planning
• Conduct patient assessments and develop individualized care plans
• Facilitate family meetings and multidisciplinary care discussions
• Arrange post-acute services, DME, SNF placements, and transportation
• Collaborate with physicians, nursing staff, and community resources
• Maintain accurate documentation using Epic EMR
• Ensure compliance with regulatory and quality standards

Requirements

Active California RN License
Minimum 2 years of recent Acute Care Case Management experience
Recent Care Coordination experience required
Previous Travel Nursing experience required
Epic EMR experience required
Strong discharge planning and resource management skills
