Family Medicine Physician (FM)
Houghton, NY We're hiring a Family Medicine Physician who wants flexible scheduling, meaningful patient relationships, and the opportunity to make a lasting impact in a mission-driven community health setting. You'll step into an outpatient primary care role providing preventive care, chronic disease management, and acute care to patients of all ages. You'll join a Federally Qualified Health Center serving a 50-town rural region across Western New York and Pennsylvania with strong clinical support, manageable patient volume, and opportunities to teach future physicians. • Flexible 4-day or 5-day clinic schedule with full-time and part-time options
• No required weekends with optional weekend shifts available
• Phone-only call approximately 5 days per month, with physicians serving as third string after hours
• Manageable patient volume of approximately 16-20 patients per day
• 1:1 staffing support to help optimize patient care
• Opportunity to perform in-office procedures based on experience and preference
• Teaching opportunity through affiliation with the UB Family Medicine Rural 1-2 Track Residency Program
• Mission-driven FQHC serving rural and underserved communities

• You hold an M.D. or D.O. degree from an accredited medical school
• You are Board Certified or Board Eligible in Family Medicine
• You hold or are eligible for medical licensure in New York

• Base salary: $205,000-$246,000 based on experience
• Sign-on bonus available
• Relocation assistance available

• Retirement: 403(b) with up to a 4% employer match
• PTO: 28 days plus 2 personal days and 7.5 paid holidays
