SMI Case Manager — In-Home & Community Support
Southwest Behavioral & Health Services
Southwest Behavioral & Health Services is seeking a SMI Case Manager for our Buckeye Outpatient Clinic to provide in-home and community-based interventions, assessments, and care coordination for adults with serious mental illness. You will collaborate with care teams, develop service plans, monitor treatment progress, and assist with transitions of care while traveling to client sites as needed.
