• Providing vital support with mobility and transferring patients

• Monitoring vital signs and reporting any changes to the Nurse and Physician

• Maintaining accurate and detailed records of patient care and progress

• Collaborating with our in-office coordinators and field case managers to ensure seamless communication and coordination with patients and families
