Social Worker: MVP Care & Community Linkage
Upstate Medical University
Upstate Medical University is seeking a dedicated Social Worker to enhance care for Multi-Visit Patients by employing evidence-based strategies in collaboration with the Emergency Department and community partners. The role entails managing and supporting at-risk patients, addressing social determinants of health, and actively participating in departmental activities. The ideal candidate must hold an MSW from an accredited program, have a minimum of 3 years relevant experience, and demonstrate strong communication and assessment skills.
