Family Practice/Primary Care Nurse Practitioner

We are currently looking for a Family Medicine nurse practitioner AND/OR physician assistant to join our Family Medicine private practice in Lincoln, CA a suburb of Sacramento.

Key Responsibilities:

• Comprehensive Primary Care: Provide preventive care including annual physicals, well-child visits, immunizations, and sick visits for both children (ages 6 and older) and adults.
• Acute Care Services: Offer urgent care, pediatric care, short-term stabilization, and post-hospital follow-up care designed to keep patients safe, independent, and out of the hospital.
• Chronic Disease Management: Manage acute and chronic conditions, including providing medication therapy management, diet and nutrition counseling, and mobility/home medical equipment assessments.
• Geriatric & Palliative Care: Provide specialized adult primary care and patient observation tailored to our senior citizen population.
• Collaborative Practice: Work collaboratively with our clinical team and network of specialists to ensure comprehensive continuity of care.
• Work Life Balance: Predictable work schedule, flexible full or part-time option, no after hours practice call.

Qualifications:

• 2+ years of primary care experience required, no new grads considered
• Must love building relationships with patients, value bedside manner, and possess excellent clinical skills
• Must comply with HIPAA rules and regulations
• State licensure and board certification is required

The salary range for this role is $70.00 per hour in base pay and exclusive of any bonuses or benefits. The base pay offered will be determined based on relevant factors such as experience, education, and geographic location.
