RN Case Manager (Home Health & Hospice) Location: South Franklin County and Madison County. Full Time. Monday–Friday, 8 AM–4 PM. Flexible schedule. On-call rotation with bonus pay. Mileage reimbursement: $0.72 per mile. Salary: $72,603–$121,005 annually. Position Summary We seek a dedicated registered nurse to oversee case management for hospice patients, coordinating individualized care plans and supporting families. Responsibilities Provide skilled nursing care and case management for hospice patients. Coordinate and develop individualized care plans. Collaborate with interdisciplinary teams to deliver high‑quality care. Maintain accurate and timely documentation. Respond to on‑call duties with optional bonus pay. Qualifications Graduate of an accredited nursing program with a current RN license. Minimum two years of RN experience; hospice or home‑health experience preferred.
