For more information, visit As a Full Time Care Manager RN, at a privately managed Health Center you'll provide accessible, comprehensive primary care services to Hormel Foods employees and their dependents in our Health Center located in Austin, MN.Schedule40 Hours Per WeekSchedule to be built during clinic hoursClinic Hours: Mon, Wed, Fri 6:30 am - 7:00 pm; Tue & Thu 7:00 am - 7:00 pmAbility to provide flexibility for both early and late shiftsWhat You'll DoDevelops personalized treatment plans using provider care plans and member preferences using a coach approach and the pillars of Lifestyle MedicineSupports a proactive, multidisciplinary team approach directed toward prevention, education and health promotionEstablishes supportive relationship with membersAssesses member's educational/clinical/social needs, prioritizing care based on elicited information from the member, Epic, and data analysisActs as resource for membersDocuments touchpoints and progress into the electronic medical recordCollaborates with the full scope of clinical team members to arrange for further medical treatment when neededProvides support and tools needed to help members take control of their healthDelivers exceptional member experiences and engaging members in their healthcare journeysImproves health outcomes and better management of any chronic conditionLowers healthcare costs while improving outcomesConsults with other members of the healthcare team to solve problems and seek adviceWorks closely with client's vendors as appropriate to support member's needs where appropriateIdentifies emergency situations and provides care within legal scope of practiceMaintains member privacy and confidential member informationParticipates in outreach/follow-up phone calls and follow-up member visitsProvides condition specific self-care education to client employee enrolled in the various condition management programsServes as member advocateActs as facilitator of Care Team meetings.Coaches effectively virtually using video and phoneCollaborates with health center staff to coordinate and promote health and wellness programsMay require other duties as assignedWhat You'll BringCurrently licensed Registered Nurse in state of practiceCurrent certification AHA or ARC Basic Life Support for health care providersBachelor's degree (BSN) from four-year college or university (preferred)At least three years' experience as a Registered Nurse caring for members with any of the following conditions: Type 2 diabetes, asthma, heart disease, heart failure or hypertension.Experience in Condition Management, Certificate in Case Management, Care Coordination, Certified Diabetes Educator (CDE) Certified Diabetes Care and Education Specialist (CDCES) a plusWork-life balance is at the foundation of how decisions are made and where Premise is headed. Premise provides its reasonable and genuinely expected range of compensation for this job of $84,000.00 - $105,000.00 annually.
