Hours: Full Time Nights 7pm-7am, weekend & holiday requirements

Job Summary:

To provide professional nursing care in accordance with the Nurse Practice Act, to collaborate with the physician and interdisciplinary team to provide patient care, to promote patient flow, and to execute the objectives and the policies of the hospital.

Accountabilities:

• Assess, plans, implements, documents, and coordinates patient care according to departmental and hospital standards and gives consideration to age-specific accommodations.
• Utilizes the nursing process for patients of all ages.
• Provides patient care, which demonstrates a comprehensive knowledge of normal physiology, disease pathophysiology, and injury states respectful of principles of growth and development over the lifespan.
• Implements evidence-based practice in a holistic manner.
• Responds promptly to physician orders. When indicated uses clinically indicated best practice order sets to guide plan of care. Evaluates and revises plan of care based on patient response to interventions.
• Demonstrates ability to perform with clinical proficiency in crises and medical emergencies.
• When assigned to the triage role, assures rapid triage assessment with assignment to treatment room. Continually communicate with the charge nurse, soliciting needed resources when in overcapacity (maximizing current space and use of best practice order sets to keep the care moving).
• Provides teaching and discharge planning based on assessment of self-care knowledge and ability on an ongoing basis.
• Involves patient/significant other in all aspects of the plan of care, serving as advocate when necessary.
• Documents all assessments, interventions and patient response in the medical record.
• Communicates and collaborates with patients, physicians, family members, and other healthcare disciplines to achieve excellence in the patient care experience during hospitalization.
• Introduces self at the time of each patient encounter.
• Contributes to the maintenance of a healthy work environment.
• Adheres to hospital policies regarding SERVICE values.
• Maintains professional appearance and follows departmental dress code.
• Promotes positive interactions with other team members.
• Speaks respectfully at all times.
• Keeps personal conversations out of patient care areas.
• Adheres to hospital policies regarding computer etiquette.

• Utilizes effective interpersonal skills when interacting with patients, family members, physicians and staff by actively listening and speaking professionally.
• Consistently and continuously communicates, consults and shares pertinent information regarding changes in patient status.
• Keeps patient informed of plan of care by offering frequent explanations related to anticipated diagnostic testing and the expected timeline related to each study and intervention. This relates to care in treatment area as well as waiting room/hallway management of patients in overcapacity situations.
• Uses interpersonal communication techniques that show a willingness to accept and give constructive feedback for the purpose of promoting professional excellence.
• Effectively communicates pertinent patient and significant other information during hand over of care according to department standards.
• Demonstrates leadership skills in team building and interpersonal relationships thus promoting a cohesive and productive environment.
• Documents appropriately inclusive of entry of problem events as indicated.
• Promotes patient safety through adherence to protocols, policies, procedures, evidence-based guidelines, The Joint Commission Standards and National Patient Safety Goals.
• Performs selected therapeutic interventions and diagnostic procedures within scope of practice and hospital policy.
• Safely administers medications and treatments as ordered according to hospital policy and formulary using the appropriate principles of safe medication administration, two (2) patient identifiers and accurate weight-based dosages.
• Safely handles various types of drugs and body fluids using knowledge of appropriate universal precautions.
• Maintains safety precautions while providing patient care including infection control isolation techniques, patient/bed positioning, side rails, restraint utilization, skin care, patient/specimen transport, physical environment, awareness, fall prevention, seizure precautions, suicide screening, and work injury prevention.
• Analyzes patient condition and responds to deteriorating clinical status by consulting physician or appropriate healthcare team members for admitted patients (inclusive of Rapid Response Team), and taking appropriate, prompt action.
• Maintains equipment in good working order, reports equipment failure, and initiates corrective action when needed.
• Protects and guarantees the confidentiality of all patient-related information, including the patient's medical record.
• Adheres to all hospital policies regarding National Patient Safety Goals; for example, consistently utilizes 2 patient identifiers before performing any diagnostic or therapeutic intervention.
• Actively participates in and supports departmental and organizational quality initiatives and goals inclusive of optimizing patient flow.
• Demonstrates appropriate skill in utilization of the computer for automated order entry, data retrieval, medication administration, nursing care planning and intervention and interdepartmental communication as determined by defined access levels.
• Keeps current in nursing practice through participation in continuing education programs.
• Identifies professional goals, which reflect own strengths and weaknesses and seeks assistance for improvement of performance.
• Demonstrates initiative and self-direction by providing care in an effective, efficient manner.
• Attends and participates in meetings according to department standards.
• Supports and participates in departmental and hospital evidence-based care quality initiatives including implementation of standard workflow for STEMI Alerts, Stroke Alerts, Sepsis identification and treatment, pressure ulcer prevention, and Infection Control initiatives.
• Participates as a team member or in workgroups on departmental or hospital evidence-based quality initiatives.
• Assist with orientation of nursing personnel.
• Maintains clinical competency according to unit specific standards.
• NIHSS certification required within 3 months of hire.
• Demonstrate competency in maintaining specific technological skills.
• Completes annual mandatory requirements as outlined in yearly departmental Staff Education Plan.
• Meets required certification/educational requirements.
• Achieving specialty certification is highly endorsed.
• Performs duties in accordance with Penn Medicine and entity values, policies, and procedures.
• Other duties as assigned to support the unit, department, entity, and health system organization.

Qualifications:

• BSN Required

Licenses, Registrations, and Certifications:

• ACLS and BLS required
• Current licensure in the Commonwealth of Pennsylvania, Department of State Bureau of Occupational and Professional Affairs.
