Virtual Primary Care NP - Home-Visit Leader

Upward Health

Upward Health is seeking a Nurse Practitioner to deliver high-quality primary care primarily through our virtual care platform, with occasional in-person support for transitions of care and wellness visits. The NP will lead a multidisciplinary team, perform risk assessments, and coordinate care for complex, chronically ill patients in a home-based setting.
