Qualifications:

• Active RN license, eligible to practice in Hawaii
• Minimum one year of recent experience in home health or case management preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and coordination abilities
• Ability to work independently and within multidisciplinary teams

Responsibilities:

• Provide exceptional patient care through coordinated case management, collaborating closely with patients, families, physicians, and the broader care team
• Lead and monitor interdisciplinary care planning, ensuring alignment with patient needs and clinical status
• Advocate for optimal resource use, reviewing treatment plans and facilitating effective care transitions
• Address complex patient needs, including those with chronic conditions, frequent ED usage, and special populations such as homeless individuals
• Work with acute care case managers to ensure seamless ongoing care
• Manage patient transfers to tertiary centers, ensuring accurate handover and continuity of services
• Oversee data collection for regulatory and accreditation compliance
• Conduct focused reviews of high-frequency emergency department patients to improve care delivery
