Registered Nurse (RN) | Home Health

Job Description

We are seeking a dedicated Registered Nurse (RN) for a full-time position in Home Health. The ideal candidate will be responsible for patient assessments, care planning, and ongoing communication with patients and their families.

General Duties

Conduct comprehensive assessments of patients' physical, psychological, social, spiritual, educational, and developmental needs.
Review patient history and physical examinations in coordination with patients and their families, ensuring completion within the appropriate timeframe.
Collaborate with healthcare team members to review information and diagnostic/laboratory data, reporting abnormal results to physicians and other caregivers as necessary.
Develop and implement a patient-specific plan of care, including discharge planning, while engaging patients, families, and the healthcare team in the process.
Set realistic and measurable goals for patients in alignment with their specific needs and the medical plan of care.

Qualifications

Required Qualifications

Experience in an acute care setting within the past 12 months.
Current nursing license in the state of Alaska.
Basic Life Support (BLS) certification; Advanced Cardiovascular Life Support (ACLS) and Pediatric Advanced Life Support (PALS) certifications may be required.

Preferred Qualifications

At least 2 years of nursing experience.

Work Schedule

Full-time position with an 8-hour day shift from Monday to Friday.
Weekend work and holiday requirements as needed.
On-call duties may be required as needed.
