The Speech-Language Pathologist (SLP) provides evaluation, treatment, and education to residents with communication, cognitive-linguistic, and swallowing disorders. The SLP works as part of an interdisciplinary care team to help residents regain or maintain their highest level of functional ability, independence, and quality of life within the skilled nursing setting.
Responsibilities:
- Evaluate, diagnose, and develop individualized treatment plans for residents with speech, language, cognitive, voice, and swallowing impairments.
- Conduct bedside swallowing evaluations and make dietary texture recommendations as appropriate.
- Provide direct therapy services and modify treatment based on resident progress.
- Document evaluations, daily progress notes, and discharge summaries in accordance with facility policy and payer requirements.
- Collaborate closely with nursing, dietary, and therapy staff to ensure coordinated care and optimal resident outcomes.
- Educate residents, families, and staff regarding communication techniques, compensatory strategies, and swallowing precautions.
- Participate in care plan meetings and contribute to interdisciplinary team discussions.
- Maintain compliance with all state, federal, and facility regulations regarding patient care and documentation.
- Supervise speech therapy assistants and students as assigned.
- Promote a positive, supportive, and therapeutic environment in alignment with the facility's mission and values.
- Other duties as assigned.
Requirements:
Education: Master's degree in Speech-Language Pathology from an accredited program. Licensure: Current and valid Speech-Language Pathologist license in the state of Oregon (or applicable state). Certification: Certificate of Clinical Competence (CCC-SLP) from ASHA preferred; CFY candidates may be considered. Experience: Previous experience in skilled nursing, subacute rehab, or geriatrics preferred. Skills: Strong clinical reasoning, excellent communication skills, team collaboration, and thorough documentation ability.
