Responsibilities Provide outpatient rehabilitation services in an in‑home setting for the older adult population. Deliver physical therapy programs to facilitate rehabilitation and address comprehensive needs of patients. Implement individualized therapy programs designed to restore, reinforce, and enhance performance. Instruct, educate, and train patients and caregivers in essential skills for promoting independence and productivity. Direct activities to promote and maintain health. Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel. Adhere to department and patient treatment schedules, documentation, billing, and related meetings. Attend patient care, staffing conferences, and other related meetings. Make referrals to disciplines and agencies for comprehensive patient care. Consult with the rehabilitation team to select appropriate therapeutic regimens. Adapt program and plan of care to individual needs. Promote a safe environment, enforce removal of hazards, order supplies and equipment as needed. Maintain equipment and report repairs. Screen, examine, evaluate patients, establish diagnosis, identify impairments, predict improvement levels, identify precautions, and design a plan of care. Develop treatment goals in collaboration with patient and caregivers, implement treatment plan, and complete documentation. Perform therapeutic exercises including aerobic conditioning, balance, agility, posture, flexibility, gait, neuromotor development, relaxation, strength, power, endurance. Perform manual therapy, airway clearance techniques, integumentary repair and protection, electrotherapy and modalities. Provide health and wellness education and chronic disease management. Assess falls and interventions to reduce fall risk, case management in home and community. Design, fabricate, fit, and train patients in adaptive devices such as orthotics and prosthetics. Select activities suited to patient’s physical, mental capacity, and interests; instruct patient in mobility techniques; evaluate progress. Perform routine reexamination and adjust treatment plan as required. Incorporate health and wellness programming into treatment plans. Provide home and community mobility assessments and recommendations for safe mobility. Perform other related duties as required. Qualifications Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility. Master’s degree in Physical Therapy, or a related field plus bachelor’s degree in Physical Therapy. Alternatively, bachelor’s degree in Physical Therapy plus five years of progressively responsible PT experience. Foreign earned PT degree evaluated as U.S. equivalent. Valid driver’s license or reliable transportation to patients’ residences. Posted salary range: USD $79,000.00 - $110,000.00 per year. #J-18808-Ljbffr
