Ability to meet productivity expectations and weekly hour guarantee as outlined by the client (weekly guarantee: 36 hours; productivity expectations: 30 points). Valid driver’s license and reliable transportation for patient visits. Mileage reimbursed between visits from first patient to last patient. Professional appearance consistent with client dress code. Preferred scrub colors: gray or black. Minimum education requirement: Doctoral preferred by client. This is client‑specific and may be required. Assignment Details Facility type: Home Health. Shift: Days. Shift hours: 08:00–16:00 (client shift schedule lists 8:00 am–4:30 pm); core shift provided as Days. Work week: 5x8 schedule. Weekly hour guarantee: 36 hours. On‑call/weekend requirement: No. Coverage area: Office service area; travel between patient homes required. Required certification: Physical Therapist license.
