Setting: Skilled Nursing Facilities, Long-Term Care Communities, and/or Home HealthSNF:Crestview Court- Cedar HillWinsor Gardens- LancasterVilla at Mountain View- DallasHH:Dallas, Duncanville, Lancaster, Cedar Hill, DeSotaPosition SummaryThe Occupational Therapist (OT) provides patient-centered occupational therapy services in Skilled Nursing, Long-Term Care, and Home Health settings. The OT evaluates patients, develops and oversees individualized plans of care, and supports patients in improving independence with activities of daily living (ADLs), functional mobility, and overall quality of life in accordance with regulatory and clinical best practices.Key ResponsibilitiesEvaluate patients and develop individualized occupational therapy plans of careProvide skilled occupational therapy treatments focused on ADLs, IADLs, cognition, strength, coordination, and functional mobilitySupervise and collaborate with Occupational Therapy Assistants (OTA/COTA) in accordance with state practice actsMonitor patient progress, modify plans of care as appropriate, and communicate changes to the interdisciplinary teamEducate patients and caregivers on therapeutic techniques, safety strategies, adaptive equipment, and home modificationsComplete timely, accurate, and compliant documentation in the electronic medical record (EMR)Ensure compliance with all federal, state, payer, and facility regulations, including HIPAAMaintain a safe treatment environment and adhere to infection control and safety protocolsDemonstrate professionalism, compassion, and respect in all patient and family interactionsQualificationsGraduate of an accredited Occupational Therapy programCurrent Occupational Therapist license in good standing (state-specific)Experience in skilled nursing, long-term care, or home health preferred (new graduates welcome to apply)Strong clinical, communication, and time-management skillsAbility to work independently while contributing to a collaborative team environmentWhy Join UsFlexible part-time scheduleSupportive leadership and collaborative care modelOpportunity to work across multiple care settingsMeaningful work making a direct impact on patient outcomesDiversity, Equity, and Inclusion are at the heart of Pivot.
