Cardiovascular Advanced Practice Provider (NP/PA) - No weekends or call Heart Care Centers of Illinois – Mokena Clinic & Affiliated Hospitals About Us: Heart Care Centers of Illinois (HCCI) is a private, physician-led cardiovascular practice that has served the Southwest Suburbs of Chicago for over 50 years. With 29 board-certified cardiovascular specialists and a presence in 10 hospitals and 6 clinics, including our Mokena clinic, we provide cutting-edge, patient-centered heart care across the full spectrum of cardiovascular medicine. At HCCI, we prioritize innovation, compassionate care, and superior outcomes. Our Mokena clinic is a key hub for outpatient care and closely collaborates with nearby affiliated hospitals to ensure seamless, high-quality treatment across inpatient and outpatient settings.
