Physical Therapist (PT) - Full-Time, Part-Time or PRN

Continuum Therapy Partners is seeking a licensed Physical Therapist (PT) to join a collaborative rehab team in a skilled nursing setting in Reedsburg, Wisconsin at Sauk County Health Care Center.

Evaluate and treat patients with mobility and functional limitations
