Key Responsibilities\n

• Conduct thorough, age- and culturally-appropriate bio-psychosocial assessments to identify mental health, substance abuse, and medical needs of clients in a primary care setting.

• Provide behavioral health services, including individual and group counseling, intake assessments, treatment planning, crisis intervention, and referrals to community resources.
\n
• Create and maintain accurate electronic documentation of patient interactions, including clinical progress notes and suicide risk assessments.
\n
• Act as a liaison between patients and local resources, assisting with medication assistance programs and connecting clients to community agencies.
\n
• Collaborate with Quality Improvement teams to enhance patient care processes and to drive data collection efforts.

Qualifications\n

Compensation and Benefits\n
