Mobile Crisis Case Manager

Come be a part of our Mobile Crisis Team and serve in the community as a Mobile Crisis Case Manager! Position Summary Provides support services and interventions face-to-face, within the home and community for individuals (children & adults) experiencing mental health or substance use-related crisis. This individual will need to be prepared to provide immediate, short-term and evidence-based practices to screen, assess, stabilize, and make appropriate referrals, including crisis stabilization units, inpatient hospitalization, and acute detoxification services. You will collaborate with the Community Support Services and other departments at PSS to support transitions of care for consumers. The Mobile Crisis Team operates 24/7 during the week- days, evenings, and night shifts, and weekend shifts available. Services are delivered in a variety of settings, including on site at the Porter-Starke Services Crisis Center and throughout the community, such as hospital emergency departments, correctional facilities, clients’ residences, and other community-based locations. A crisis may be defined as substance intoxication or overdose, serious mental illness or serious emotional disturbances, suicide attempts and ideation, other risk of harm to self, risk of harm to others, disruptive behavior, domestic violence, and other crises as defined by the individual. Uses a trauma-informed, strength-based approach and de‑escalation techniques to lower risks and resolve the crisis if possible so that a higher level of care is not needed. Prepared to provide immediate, short‑term and evidence‑based practices to screen, assess, stabilize, and make appropriate referrals, including crisis stabilization units, inpatient hospitalization, and acute detoxification services. Completing outreach attempts to provide follow‑up referrals that ensure locating, coordinating, and monitoring necessary behavioral health, physical health, and addictions care and services. Collaborates with Community Support Services and other departments at PSS to support transitions of care for all consumers. Therapeutic interventions for individuals diagnosed with serious mental illness including de-escalation techniques. Signs and symptoms of mental health disorder Crisis intervention services on‑site and off‑site, face‑to‑face, within the home, and community Assessing the client’s strengths and mental health needs accurately and objectively Providing selected therapeutic communication to clients Client’s rights and confidentiality Typing and basic computer usage. The requirement of adhering to policies and procedures of the agency and professional standards. The Staff Member Must Demonstrate Skills In Establishing trust between clients and treatment team members. Providing information and referrals to community services both in person and via the phone Assisting in implementation of goals and objectives identified in treatment plans Interacting with clients as peers by sharing common experiences. Assisting clients in understanding and managing their disorder and symptoms. Assisting clients in connecting with self‑help groups when appropriate. Assisting clients in gaining access to local programs and services. Clarifying clients’ understanding of their rights and responsibilities as a recipient of substance use disorder services. Completing all necessary as required Maintaining strict confidentiality at all times. Communication styles that generate a positive work environment. Locating, coordinating and accessing appropriate services for clients. Essential Functions Must be adaptable and able to work various shifts, including days, evenings, nights, weekends, and holidays. Completion of required assessments screenings, including but not limited to the Columbia Suicide Rating Scale (CSSRS). Engage individuals in the crisis planning process and completion of a safety plan. Build rapport to support increased engagement, instillation of hope, and care coordination. Follow‑up with individuals in‑person, via phone, or telehealth within two days of the initial crisis intervention Provide opportunities for warm hand‑offs to identify resource needs (insurance navigation, housing, benefits and entitlements, primary care, educational, or vocational supports) Provide referral to medication management to monitor medication compliance, education and training about medications, monitoring side‑effects, and providing other nursing/medical assessments. Provide referral to Case Management (children’s or adult) when appropriate Act as informant and advocate to clients regarding consumer rights and responsibilities. Develop and maintain close working relationships with management staff, co‑workers and peers. Participate in team staffing to provide client and peer insight. Collaborates with treatment team to increase patient engagement in treatment as evidenced by consistent attendance at huddles, weekly team meetings, staffing, clinical supervision, and with community partners.
