Home Health Rn/Lpn

Home Care Now is seeking a compassionate and clinically strong MALE Home Health RN/LPN to provide one-on-one care in a home. This role is ideal for a nurse who values autonomy, patient relationships, and the ability to deliver skilled care in a home-based setting rather than a facility environment.

Travel to patient homes to provide skilled nursing care

Assess patient condition, vital signs, and response to treatment

Administer medications

Complete accurate, timely documentation for each visit

Active RN license in the state of employment

Current CPR or BLS certification

Level 2 Background

Strong assessment and communication skills

Ability to work independently in a home-based care setting

At least 1 year of Home health experience

Experienced with trach g/j-tube

Both days and Nights available 7am to 7pm/7pm to 7am
