Annual Salary: $100,725.73 - $142,225.00 (based on experience; 2,080 hours/year)

Schedule: Monday - Friday

Sign-On Bonus: $25,000 for a 3-year commitment

Arizona PA License (required)

DEA Registration (reimbursed)

Board Certification (reimbursed)

Professional Liability Insurance: fully covered

License & Subscription Fees: reimbursed

Paid CME: 5 days/year + $4,500 annual stipend

Paid Time Off: 20 vacation days + 10 sick days/year
