As the Harm Reduction Case Manager , you will play a pivotal role in individuals' lives, making a significant impact through:

• Conduct outreach activities to engage the target population
• Identify community partners to engage PWUD who are unaware of their status or untreated
• Implement harm reduction strategies aimed at helping individuals minimize their immediate health and safety risks
• Provide enrollment into the agencies Syringe Exchange Program
• Facilitate brief assessments with the target populations
• Through review of the assessment, identify community based services to provide client linkage
• Facilitate linkage to services through escort as necessary

Harm Reduction Case Manager Minimum Salary: $23.08

Harm Reduction Case Manager Maximum Salary: $23.08

Harm Reduction Case Manager Schedule: Mon - Fri 8:30am - 4:30pm

Qualifications:

• Associate's degree
• 1-3 years of relevant experience, Human Services
• Personal Cellphone
• Valid driver's license, proof of current car insurance and registration and access to a reliable, working vehicle
