Tufts Medicine Care at Home is seeking an experienced registered nurse based in Melrose to provide professional and nonprofessional nursing care in alignment with physician orders and hospice guidelines. The role focuses on patient assessments, admissions to Hospice, and ensuring continuity of care from hospital intake to ongoing home hospice services.
