This rewarding role offers the opportunity to directly impact the lives of individuals with developmental disabilities while building a fulfilling career in human services.We are hiring for all office locations:StocktonModestoTracySan Andreas (serving Tuolumne, Amador, and Calaveras counties)Note for San Andreas office: Your caseload location (Tuolumne, Amador, or Calaveras) will be based on where you reside.What You'll Do:Champion for Others: Advocate for the rights and needs of individuals with developmental disabilitiesConnect People to Resources: Coordinate essential services that enhance quality of lifeBuild Meaningful Relationships: Work directly with consumers, families, and community partnersKey Duties for the Service Coordinator Position:Case Management & Planning Assess, coordinate, and implement individual program plans through team processesSchedule and facilitate various reviews and team meetingsDocumentation & Reporting Complete comprehensive case documentation including program plans, evaluations, progress reportsPrepare service authorizations, court reports, and Medicaid documentationAdvocacy & Support Provide advocacy, education, and crisis intervention for clients and familiesServe as liaison between clients and various service providersData Collection & Analysis Collect and report client statistical data for various surveys and assessmentsProfessional Collaboration Participate in internal/external committees for education and agency improvementMaintain positive communication with partner organizationsEmergency Response Serve as Officer of the Day and participate in After Hours Response System as assignedAreas of SupportThis position may support individuals in the Children or Transition/Adults programs, depending on assignment and organizational needs.Why You'll Love Working With Us:Diverse Work Environment: Split your time between our welcoming office and out in the community, hybrid schedules availableNever a Dull Moment: Each day brings new challenges and opportunities to problem-solveProfessional Growth: Ongoing training to expand your knowledge and skills in developmental servicesJoin a Team That Makes a Difference!As a Service Coordinator, you'll be the vital link between the individuals we serve and the resources they need to thrive.
