Work Environment\nThis position allows you to work remotely while engaging with clients within their homes or through our online platforms. You will be part of a dynamic team that includes healthcare professionals, caregivers, and family members, fostering a collaborative atmosphere aimed at enhancing the well-being of seniors. Scheduled work hours are from Monday to Friday, 8:00 AM to 4:30 PM, although flexibility will be necessary to accommodate the varying needs of our clients. On-call availability may be required to address urgent or unexpected situations.
Key Responsibilities\n- \n
- Conduct comprehensive assessments of seniors’ psychosocial needs, assessing emotional, social, and mental health factors that affect their quality of life. \n
- Work closely with clients and their families to design individualized care plans that reflect their specific preferences and goals. \n
- Provide counseling and emotional support, helping clients navigate significant life transitions, such as end-of-life considerations, and providing grieving support for families. \n
- Advocate for client rights, connecting them to vital community resources and ensuring they receive the necessary services and support. \n
- Collaborate with a team of healthcare providers—including nurses and physical therapists—to coordinate care plans that incorporate all aspects of the client’s well-being. \n
- Regularly meet with clients to monitor their mental and emotional states, providing adjustments to care plans as needed, and addressing any emerging safety or quality of life issues. \n
- Educate families about the available resources, support groups, and health programs that promote social engagement and meaningful experiences for seniors. \n
- Ensure proper documentation of all assessments, interactions, and progress notes to maintain accurate records in our electronic health record (EHR) system. \n
- Stay informed on best practices, emerging trends, and policy changes in social work ethics and regulations, ensuring compliance in all interactions. \n
Your primary focus will be seniors experiencing various life transitions associated with aging, particularly those requiring hospice care. You will work with clients at sensitive stages in their life, providing support that empowers them while also addressing the needs of their families during challenging times. Your role is crucial in improving not just the emotional health of the clients, but also that of their loved ones as they navigate through end-of-life care considerations.

- Possession of a Master’s degree in Social Work from an accredited institution. \n
- Current state licensure as a Licensed Master Social Worker (LMSW) is mandatory. \n
- A minimum of two years of hands-on experience in social work, particularly in community or hospice settings. \n
