Job Title: Registered Nurse Department: Elderone Eastside Homecare Location: 150 French Road Rochester, NY 14618 Hours Per Week: 40 hours Schedule: M-F 8am-4:30pm 4 weekends per year SUMMARY As a Registered Nurse in Home Care, your exceptional ability to teach, counsel and empathize with each patient leads to an enhanced quality of life. RESPONSIBILITIES Patient Care: Conduct initial assessments; establish plan of care and provide prescribed treatments and demonstrated nursing abilities; coordinate the delivery of 24-hour care; Communication: Counsel and guide individuals and families towards self-help in recognition and solution of physical, emotional and environmental problems; Evaluation & Documentation: Conduct reassessment of each patient on a regular basis; maintain complete and accurate clinical documentation; supervise and serve as a resource to all home care staff; REQUIRED QUALIFICATIONS Nursing Diploma required, ASN preferred. License and current registration to practice as Registered Professional Nurse in New York State. Current NYS Driver’s license. PREFERRED QUALIFICATIONS Bachelor's degree of Science in Nursing preferred. 1 year of experience working with a frail or elderly population preferred. Availability of Motor Vehicle. 6 months-1 year experience conducting UAS-NY Community Assessment preferred. EDUCATION AS: Nursing (Required) LICENSES / CERTIFICATIONS BLS - Basic Life Support - American Heart Association (AHA); RN - Registered Nurse - New York State Education Department (NYSED) PHYSICAL REQUIREMENTS M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting. PAY RANGE $75,733.00 - $101,014.00 CITY Rochester POSTAL CODE 14618 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
