Quality Improvement Specialist

Role: Senior Quality Improvement Specialist Responsibilities Lead clinical quality initiatives across home and community-based care programs. Analyze clinical outcomes and identify performance gaps. Design and implement data-driven interventions to enhance patient safety, care quality, and regulatory compliance. Collaborate with interdisciplinary teams to develop and monitor quality metrics. Support accreditation processes and conduct audits. Provide education on best practices to staff. Ensure patient-centered, high-reliability care aligned with organizational standards and mission. Qualifications & Skills Strong analytical skills to interpret clinical data and outcomes. Experience in quality improvement methodologies and initiatives. Ability to collaborate effectively with diverse teams. Knowledge of regulatory standards and accreditation processes. Excellent communication and educational skills.
