Job Summary Provide one‑on‑one skilled nursing care to pediatric and adult clients with chronic, complex, or medically fragile conditions in their homes. Collaborate with physicians, therapists, case managers, and family members to support the client’s individualized plan of care. Deliver continuous hourly care, ensuring safety, comfort, and clinical stability throughout each shift. Qualifications Must have a valid Indiana nursing license in good standing. A minimum of one year of nursing experience as a Registered Nurse. Current CPR certification (Free re-certification with Adaptive). Good organizational and communication skills.
