Job Summary

Responsibilities

• Provide direct patient care to residents in our skilled nursing facility, including administering medications, wound care, and conducting vital sign checks

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Monitor and report changes in resident condition to the healthcare team and physicians

• Maintain accurate and up-to-date records of patient care and treatment

• Collaborate with therapy staff to ensure successful rehabilitation outcomes

• Participate in quality improvement initiatives to continuously improve patient care and satisfaction
