Description:
• This position involves providing behavioral health case management services in a primary care clinic setting.
• The role is full-time and located onsite in Jackson Heights, NY, with standard weekday hours.
• The successful candidate will use various technology platforms to manage patient records and coordinate care.

Requirements:
• NYS Licensure with LMHC or LMSW required
• Advanced degree in mental health counseling or social work.
• Two years of work experience in a related field.
• UAS-NY certification within one month of hire.
• Experience working with diverse populations.
• Excellent written and verbal communication skills in English.
• Ability to work flexible hours including some evenings and weekends.

Qualifications:
• NYS Licensure with LMHC or LMSW required
• Proven experience in behavioral health case management.
• Strong crisis intervention and counseling skills.
• Ability to coordinate and link patients with additional services.
• Experience in maintaining accurate patient records.
• Proficiency in using various technology applications.
• Fluency in Spanish or an Asian language is a plus.
• Excellent customer service skills.
