Bienvivir All-Inclusive Senior Health ("Bienvivir") is a community-based, patient-centered, comprehensive health care delivery system that advocates and promotes quality of life, optimum independence, dignity, and choices in a nurturing environment for frail seniors. Since 1987, Bienvivir has served the frail seniors of El Paso, Texas through the provision of the Program of All-Inclusive Care for the Elderly ("PACE").

PACE is a unique managed care benefit for frail seniors (referred to as participants) age 55 and older who are certified by the state as needing nursing home level care and who reside in a PACE service area. PACE programs coordinate and provide comprehensive medical and support services so that participants can remain independent and stay in their homes for as long as safely possible.

LVN Treatment Nurse

GENERAL DESCRIPTION The Treatment Nurse works under the direct supervision of the Clinic Manager in the daily operation of the medical component of the Adult Day Care Center. The treatment LVN's duties will include monitoring and maintaining skin integrity, health education of participants and families, the provision of treatments as per the treatment plan and as per the physician's orders in the medical record.

RESPONSIBILITIES

• Provides treatments and patient care as appropriate and as ordered by the PCP under the Treatment Administration Record under the supervision of the Clinic Manager or the PCP.
• Works closely with the Wound Care Nurse Practitioner to follow up on participants as scheduled.
• Conducts home visits to provide treatments and focused assessments in the participants place of residence. Provides report on findings to the Participant Manager/PCP.
• Conducts monthly medication reviews on all assigned participants and reports any abnormal findings to the Participant Manager/PCP for follow up.
• Documents all participant care provided in the EMR.
• Provides education and supplies necessary to carry out wound care treatments to participants and caregivers for treatments done at home.
• Ensures adequate nursing supplies required for treatments in the center, as well as those done at place of residence.
• Maintains the wound care log, wound care checklist and foley tracking tool.
• Participates in daily IDT meetings as necessary.
• Provides weekly report on wound status in care planning meetings.
• Provides coverage for Medication and Consult nurses as needed.
• Performs other duties as assigned by the Clinic Manager.

QUALIFICATIONS/REQUIREMENTS

• A graduate of an accredited nursing program with a license to practice in the state of Texas as a Vocational Nurse.
• One year of experience in treatment, with a frail or elderly population preferred.

• Must possess knowledge of nursing theory and practice.
• An individual with a strong knowledge base and keen understanding and appreciation of the elderly.
• The ability to present to the community the philosophy of community-based long-term care for the elderly.
• The skill to work collectively and communicate effectively with staff, participants, and family members.
• Adaptable individual with ability to work with the interdisciplinary team approach to care for the elderly.
• The ability to accept supervision in an appropriate manner.
• Proficient in computer skills.
• Bilingual preferred.
