Astera Health is looking for a full-time Physical Therapist to join our Rehabilitation Services team.

This position is scheduled for 80 hours per bi-weekly pay period, Monday-Friday in Henning with inpatient weekend rotation every 7-8 weeks in Wadena. Responsibilities required, but not limited to:

• Ability to operate independently at the Henning PT clinic, while maintaining strong communication and collaborative relationships with the broader Astera team to ensure consistency of care and culture
• Formulates total treatment programs, adjusts program to each patient's needs and condition to ensure a positive outcome.
• Carries out treatments in accordance with physician's orders. Contacts physicians as needed concerning patient condition or progress to maintain continuity of care.
• Documents evaluation and progress notes for each patient treated.
• Follows established policies, procedures, and safety precautions with use of modalities and equipment to prevent injury.
• Provides patient care in a safe manner during treatments.
• Provides physical therapy services as requested by Astera Health to meet outreach needs. Complete all documentation in adherence to Astera Health and CMS standard with 24 hours of treatment session
• Will work with in the rural general setting for treatment in areas of orthopedics, athletes, and neurological therapy across all ages.

Education and Experience Required:

• Graduate of accredited Physical Therapy Program
• Licensed Physical Therapist in the State of Minnesota
• 3+ years of physical therapy care experience to perform the essential duties of this position.

Pay or shift range: $37.05 USD to $55.57 USD
